Revenue, cost, cash flow and profit are not interchangeable

Include empty miles and percentage fees deliberately
Loaded RPM spreads revenue over paid miles. True RPM includes loaded and empty miles, because deadhead still consumes fuel, maintenance capacity, tires and time. Dispatch and factoring fees behave differently from CPM: they usually rise with gross revenue, so break-even and target calculations must account for the retained share of each revenue dollar.
